We do offer telemedicine appointments to estabilished patients for appropriate medical concerns (some concerns require hands-on examination). If you are interested in this option let us know in your request.

(1) PLEASE VISIT THE FORMS PAGE, PRINT OFF AND FILL OUT THE RELEVANT FORMS FOR YOUR APPOINTMENT TYPE.
(2) EITHER BRING THE COMPLETELY AND THOROUGHLY FILLED OUT FORM(S) TO YOUR APPOINTMENT OR SEND A PICTURE OF THE COMPLETED FORM(S) VIA TEXT TO 912-343-7675.
THIS WILL GREATLY REDUCE WAIT TIME FOR YOUR APPOINTMENT.
